We review the referral details, including the young person’s background, support needs, and risk assessments.

Planning the Transition

We coordinate with social workers, families, and other professionals to create a smooth and supportive transition plan.

Placement & Welcome

The young person moves in and is welcomed by staff. A key worker is assigned, and the initial support plan is put in place.
